The Job

The Department of Public Health is recruiting for Community Health Workers (CHW)* - Contract who assist and support clients in clinical and non-clinical settings. A CHW is the bridge of support for individuals in the community, to improve health understanding, access and outcomes. CHWs will establish a trusting and safe connection with clients for the purpose of assisting in navigating through provider care plans, client's treatment plan and, shared care plans.

This is a contract position which does not attain regular status as a County employee. Contract positions serve at the pleasure of the appointing authority.

CONDITIONS OF EMPLOYMENT



Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.

Travel: A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Mileage reimbursement and proof of automobile liability insurance must be maintained.

Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

Visa Sponsorship: San Bernardino County is not able to consider candidates who will require Visa sponsorship at the time of application or in the future. Candidates must be able to present their legal right to work in the United States at the time of employment.

CHW Certification: Incumbents must successfully complete a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of hire or be terminated.

Minimum Requirements

Education: GED or high school diploma.

AND


Experience: One (1) year of experience providing healthcare, mental health, social or human services. Experience must include one of the following: performing needs assessments, conducting interviews, referring clients to services or performing crisis interventions.

Desired Qualifications

The ideal candidate will possess the following knowledge/skills/abilities:

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
