The Job

The Department of Public Health is recruiting for Community Health Workers (CHW)* - Contract who assist and support clients in clinical and non-clinical settings. A CHW is the bridge of support for individuals in the community, to improve health understanding, access and outcomes. CHWs will establish a trusting and safe connection with clients for the purpose of assisting in navigating through provider care plans, client's treatment plan and, shared care plans.

Duties include, but not limited to

• Facilitating referrals and appropriate paperwork for clients to specialty clinics and treatment centers.
  
• Consulting with physicians and other members of the care delivery team on the client's treatment plan.
• Promoting and providing education and awareness related to the treatments and services provided to clients, families, staff and the community in a culturally appropriate manner.
• Promoting and providing resources and support to clients for substance use disorder, violence awareness and prevention, preventive healthcare, chronic disease management and sexual reproductive health.
• Operating and maintaining mobile testing van, including setup and takedown at field testing sites and community events.
• Tracking client outcomes and relevant data using manual and automated systems.
• Guiding clients in the application process for financial healthcare coverage.
• Building and maintaining a positive and professional working relationship with healthcare teams, public health staff, and external partners.
• Ensuring documentation is accurate, pertinent and in compliance with department protocols and policies.
• Assisting clients with scheduling appointments and navigating social service systems as indicated by the shared care plans.
• Recognizing and reporting signs and symptoms of abuse and neglect to providers. Completing all mandatory reporting requirements, as needed.
• Conducting outreach and supporting clients in their homes, community, or clinic setting.
• Maintaining confidentiality of Protected Health Information per the Health Insurance Portability and Accountability Act.

This is a contract position which does not attain regular status as a County employee. Contract positions serve at the pleasure of the appointing authority.

CONDITIONS OF EMPLOYMENT



Pre-Employment Process: Prior to appointment, applicants must pass a background investigation which includes verification of employment history and education, fingerprinting, physical exam, and drug screening.

Travel: A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Mileage reimbursement and proof of automobile liability insurance must be maintained.

Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

Visa Sponsorship: San Bernardino County is not able to consider candidates who will require Visa sponsorship at the time of application or in the future. Candidates must be able to present their legal right to work in the United States at the time of employment.

CHW Certification: Incumbents must successfully complete a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of hire or be terminated.

Minimum Requirements

Education: GED or high school diploma.

AND

Experience: One (1) year of experience providing healthcare, mental health, social or human services. Experience must include one of the following: performing needs assessments, conducting interviews, referring clients to services or performing crisis interventions.

• Be culturally sensitive to the population being served.
  
• Any combination of lived experience working with diverse populations in the community (e.g., women with infants and young children; persons experiencing housing instability, food insecurity, unemployment, interpersonal violence, neglect, racism, discrimination, HIV/AIDS, substance abuse; persons involved in the justice system; older adults, underserved populations; or other circumstances that could prevent a person from achieving their optimal wellness).
  
• Possess basic understanding of medical terminology.
  
• Possess general knowledge of Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint).
  

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
