Caseworker Specialist IFT / PT Status – Full TimeSalary – $23.86 - $35.85 Hourly RateRemote Work / In-Office – HybridKey ResponsibilitiesServeas the initial point of contact for guardianship inquiries by screening callsand email referrals, collecting key information, and determining eligibilityand priority level.Assesscurrent and potential client/patient needs and also develop and implement plansto meet those needs.Monitorclient funds, which includes managing client financial accounts or budgetactivities and providing fiscal information, and preparation of annual courtaccounting reports.Applyfor and maintain public benefits eligibility.Preparedetailed case documentation including opening and closing cases, intakerecords, investigation summaries, case histories, activity logs, risk/needsassessments, social summaries, service plans, reports to the court, andcorrespondence, etc.Provideclients with information concerning rights, options, benefits, services, goalsand expectations.Conductmonthly visits with clients and complete documentation.Collaborateand coordinate with service and medical providers.Performfield visits and determine the need for guardianship and/or health andwell-being of an adult.Coordinatewith and refer to community resources, placement providers, collateralagencies, the court, and/or other community partners for clients receivingservices.QualificationsCanuse independent judgment and discretion.Beproactive.Hasexperience assessing physical, mental, social, or economic needs.Is detail oriented.Canmaintain objectivity.Candeal with situations with patience, tact, and professionalism.Canread, interpret, and apply laws, rules, regulations, policies and/or procedures.Canwrite comprehensive reports that are concise and understandable on financialtransactions, social interactions, and medical conditions and adherence to careplans.Uselogic to analyze or identify underlying principles, reasons, or factsassociated with information or data to draw conclusions.Useinvestigative techniques and processes.Canwork with and around potentially high risk mentally and emotionally unstableadults.Isable to handle exposure to substandard living areas including animal waste,vermin, odors, etc.Preferred or trainable/on-the-job qualifications:Hasfour years of full-time education or professional work experience related tothe care of vulnerable adults, including elderly adults and adults withdisabilities or mental illnesses.About the RoleLocation – 288 North 1460 West, Salt Lake City, UT 84116Background Check Required – You must successfully pass a criminal history check.Drug Test Required – NoSchedule Code – B - Competitive Career Service - Employment in this position requires a probationary period.Why You Should Join Our TeamIn Utah, we believe hard work is important, but balance is essential.
