Community Transition Coordinator/SpecialistThis position is not a work-from-home or desk-only position.Applicants must :Have reliable transportation and be willing to travel throughout all 15 NWGA counties.Be comfortable visiting consumers' homes, nursing facilities, schools, businesses, and community events.Be proficient with Microsoft Office 365, email, online databases, and electronic documentation.Be capable of managing a full caseload independently.Successfully pass a criminal background check.Applicants who do not meet these minimum requirements should not apply.Position SummaryNWGA Center for Independent Living is seeking a professional who is dependable, self-directed, and committed to serving individuals with disabilities. This position requires professionalism, accountability, and the ability to manage a demanding workload while representing the organization throughout the community.Primary ResponsibilitiesAssist individuals transitioning from nursing facilities into the community.Coordinate housing, equipment, benefits, and community resources.Work closely with hospitals, nursing facilities, waiver providers, and community partners.Maintain accurate WellSky and agency documentation.Advocate for consumers throughout the transition process.Travel extensively throughout Northwest Georgia.Minimum QualificationsBachelor's degree or relevant human services experience.Knowledge of community resources and durable medical equipment preferred.Reliable transportation and valid driver's license.Strong documentation and computer skills.Background check required.Professional ExpectationsMaintain confidentiality and professional conduct at all times.Meet deadlines and complete documentation accurately.Communicate effectively with supervisors, coworkers, and community partners.Represent NWGA CIL professionally in the community.Failure to meet performance expectations may result in disciplinary action.Contact Information : [email protected]#J-18808-Ljbffr
