The Social Worker performs social work duties for County residents related to public health clinics and community health initiatives. Work involves coordinating and providing services to clients in completing forms and referral to other social service agencies. Employee is also responsible for maintaining positive relationships with other social service agencies.
Examples of Duties Coordinates and assists clients in completing eligibility forms.
Interviews clients for possible referrals to other social service agencies.
Maintains positive relationships with other social service agencies.
Assists with patient follow-ups and completion of reports when necessary.
Responds to and evaluates social needs of clients in conjunction with Denton County Public Health clinic services and programs.
Conducts assessment of social determinants of health and coordinates appropriate referrals.
Serves as staff lead for agency behavioral health initiatives.
Provide psychosocial support and assistance in resource navigation.
Provides assistance to the Preparedness and Response Division during public health crisis events.
Regular and punctual attendance is required.
Performs other related duties as required.
Minimum Qualifications Bachelor's degree in related field and a minimum two years of experience in a related field; or an equivalent combination of education and related experience required.
Current Social Work license in the State of Texas is required.
Possession of a valid driver's license. Must be able to obtain a State of Texas driver's license within 90 days of employment.
