Under general direction or direct supervision, plans, develops, and coordinates a variety of community health nursing services, including special programs such as the HIV/AIDS Program or Communicable Disease Surveillance and Case Management, STD Surveillance, Immunization Assistance Program (IAP) Clinical Programs and Case Management, and the Tuberculosis Program. This involves promoting and preserving community health through nursing services, health education, and consulting services, including disease prevention and control and health awareness promotion. Additionally, responsibilities may include providing lead direction, coordinating work, and training other professional nursing and support staff, as well as developing and administering program budgets.

Minimum Qualifications

COMMUNITY HEALTH NURSE I

Licensing and Certifications:

Possession of, or ability to obtain, an appropriate valid California Driver’s License.

Possession of a valid license as a Registered Nurse in California.

Certain positions may require a School Nurse Credential.

Must be eligible for State Skilled Professional Medical Personnel (SPMP) classification as defined by 42 CFR PART 432.

Education and Experience:

Completion of sufficient education and experience to meet State of California certification requirements.

Some nursing experience in a community health setting is highly desirable.

Additional directly related experience and/or education may be substituted.

COMMUNITY HEALTH NURSE II

Licensing and Certifications:

Possession of, or ability to obtain, an appropriate valid California Driver’s License.

Possession of a valid license as a Registered Nurse in California.

Must be eligible for State Skilled Professional Medical Personnel (SPMP) classification as defined by 42 CFR PART 432.

Certain positions may require School Nurse Credential.

Education and Experience:

One (1) years of full-time community health nursing experience comparable to a Community Health Nurse I with the County of Lake.

Completion of sufficient education and experience to meet State of California certification requirements.

COMMUNITY HEALTH NURSE, SENIOR

Licensing and Certifications:

Possession of, or ability to obtain, an appropriate valid California Driver’s License.

Possession of a valid license as a Registered Nurse in California.

Must be eligible for State Skilled Professional Medical Personnel (SPMP) classification as defined by 42 CFR PART 432.

Education and Experience:

Completion of sufficient education and experience to meet State of California licensing requirements.

Associate degree from an accredited college or university in Nursing, preferably including completion of an accredited public health nursing program.

AND

Two (2) years of community health nursing experience comparable to a Community Health Nurse II with the County of Lake.

Additional directly related experience and/or education may be substituted.

RETIREMENT - County employees are members of the California Public Employee Retirement system (CalPERS).CalPERS provides a defined benefit pension at retirement for County employees.The pension formulas and employee contribution amounts may vary by bargaining unit, date of hire, prior qualifying service, and classification. HEALTH AND INSURANCE BENEFITS - Medical, dental, hospital, vision, life and disability coverage is available to employees and their dependents. The County contributes $1,500 of the monthly premiums for the insurance plan options.

VACATION ACCRUAL may be adjusted for current city or county government employees.

VACATION - 80 hours per year during the first five years of service, 120 hours per year after five years, and additional accruals based on years of service beyond fifteen years. Pro-rated for part-time employees.

SICK LEAVE - Accrues at a rate of 96 hours per year. Pro-rated for part-time employees.

HOLIDAYS- An average of twelve paid 8-hour holidays per year. Pro-rated for part-time employees.

SALARY - Lake County has an adopted salary schedule. The salary range for most classifications is in five increments with a provision for longevity increases. Appointments are normally made at the first step of the salary range.

SOCIAL SECURITY - County employees participate in Social Security.

CALIFORNIA STATE DISABILITY INSURANCE - SDI is available to some County employees.

DEFERRED COMPENSATION PLANS and CREDIT UNION MEMBERSHIPS are available to County employees.
