Description:Jefferson County Human Services is seeking
Bilingual Long Term Care (LTC) Intake Case Managers responsible for the member-facing work of this division. Depending on the focus of their team,
Bilingual LTC Intake Case Managers provides comprehensive case management which involves providing individualized support and coordination of services for members, often with complex needs. This includes conducting assessments to determine eligibility, linking members to appropriate resources, and maintaining ongoing communication to monitor progress. It requires a thorough understanding of community resources, policies, and regulations, as well as the ability to advocate effectively for members.
Experience in a high-volume customer service position demonstrates the ability to manage multiple tasks simultaneously while maintaining a professional and empathetic demeanor. This includes resolving member concerns promptly, addressing inquiries accurately, and handling challenging situations with diplomacy and patience. It highlights skills in communication, problem-solving, and the ability to thrive in a fast-paced, member-focused environment.

Bilingual LTC Intake Case Managers for the following areas:
IDD (Intellectual and Developmental Disabilities) - The intake case managers in this area are responsible for determining the functional eligibility of members with Intellectual and Developmental Disabilities and their families.
Non-IDD - The intake case managers in this area are responsible for determining the functional eligibility for members experiencing needs related to physical disability, chronic illnesses, mental health, and aging.

Schedule: The county operates primarily on a 4 day/10-hour work week. (Monday-Thursday); Human Services works a mix of 4 day/10-hour or 5 days a week (Monday-Friday). This position is hybrid, with remote work and in-office work as determined by team and agency needs.
Hiring Range:$56,869.00 - $67,362.00 Annually Compensation will be determined based on education, experience, and skills.
Bilingual proficiency in Spanish is an incredible asset to this team and will be compensated upon completion of communication skill test.

JOB RESPONSIBILITIES Bilingual LTC Intake Case Managers have the following responsibilities:
- Determine member eligibility by gathering collateral information from hospital, nursing home, long-term skilled home health and home-based services.
- Complete a face-to-face assessment of members.
- Attend court appeal hearings to defend denied applications.
- Prepare all legal documents for the appeal hearing.
- Work with various Medicaid programs (e.g., Home and Community Based Services (HCBS), Elderly, Blind, and Disabled, Community Mental Health Services, Complimentary and Integrative Health Waiver, Brain Injury, Children’s HCBS, Program of All Inclusive Care of the Elderly, Home Care Allowance, and Nursing Home programs).
- Maintain member care plans by reassessing as necessary.
- Ensure family members are fully informed of options available.
- Take and make referrals as necessary.
- Consult with state agents.
- Other duties and responsibilities as assigned.

Minimum qualifications:
- Bachelor’s Degree in a related field and;
- One (1) year of work-related experience in comprehensive case management and a high-volume customer service position.
- Or an equivalent combination of education and experience.
Preferred Qualifications:
- Bilingual in Spanish highly preferred.
- Knowledge of laws relating to children and families.
- Experience with Microsoft Suite Office.
Knowledge, Skills and Abilities:
- Excellent individual, group and family crisis management skills.
- Able to learn more than one complex computer system and is patient in the learning process.
- Ability to problem solve by using critical thinking and utilizing resources/researching to find solutions that are best for the member/family and the department.
- High level organizational and time management skills and thrives with strict deadlines.
- Strong customer service and interpersonal skills with the ability to adapt communication based on level of program knowledge of the audience.
