Role Description

This position will work under the direct supervision of a Risk Prevention Area Lead to assess and address barriers that result in at-risk outcomes for people and providers with specialized needs. This includes assisting and overseeing the high-risk transition process, including the sex-offender and forensic process; managing the consumer referral database; providing training and support to providers, including residential, behavioral, and support coordination entities for both the transition process and other risk prevention efforts as they are built. This function will also assist in further development of risk prevention efforts, and participate in statewide and area Tiered Supports trainings and meetings. Consultation and assistance will be remote when possible. However, training, meetings, and job duties may require travel at times.
