What does this position do?In the role of Community Health Worker for the County of Los Angeles, you gain an outstanding chance to assist professional staff in providing direct health, mental health, or social services to clients and patients.
Essential Job Functions- Receives referrals and reviews referral information to provide release planning services and introduce the Patient Reentry Care Management (PRCM) program to incarcerated individuals, that may include individuals with mental illness and/or dual diagnosis history.
- Collaborates with the client and PRCM care team to complete and review the Initial Release Plan (IRP). This is based on the client’s service area.
- Uses various county applications (Los Angeles County Sheriff's Department (LASD), ORCHID, Integrated Behavioral Health Information System, Court Portal, etc.) to research client historical information, location of client, court date, housing, health status, and level of observation.
- Documents client encounter in online medical chart such as ORCHID or other client tracking spreadsheets or systems.
- Gathers and uploads client documents into various systems for the purpose of ensuring confidential documents are completed.
- Places resources in client’s property, e.g., Transit Access Pass (TAP), Department of Motor Vehicles Voucher, IRP, Substance Abuse Service Helpline (SASH) Flyer, Justice Care and Opportunities Department (JCOD) Flyer, and/or other LA County Resources.
- Collaborates with LASD and Correctional Health Services personnel for assistance and orientation with the continuum of client care in a jail setting.
- Provides information on jail resources, as needed.
- Assists with program development and training new PRCM staff as needed.
- Assists with other care team tasks as needed.
Requirements SELECTION REQUIREMENTS :
One year of full-time experience working with the public or with community groups performing duties such as interviewing clients or patients concerning health or social service matters, answering questions
, and providing information about health, mental health, and social services to clients or patients
SPECIAL REQUIREMENT INFORMATION:* Experience is evaluated on the basis of a verifiable 40-hour workweek. Work performed part-time will be prorated on a month-for-month basis.
LICENSE :
A validCalifornia Class C Driver License or the ability to utilize an alternative method of transportation to carry out job-related essential functions.
PHYSICAL CLASS II - Light :
This class includes administrative and clerical positions requiring light physical effort that may include occasional light lifting to a 10 pound limit and some bending, stooping, or squatting. Considerable ambulation may be involved.
DESIRABLE QUALIFICATIONS :
Additional credit will be given to applicants who possess the following desirable qualifications:
- Experience working with individuals involved with the justice system such as probation, parole or re-entry.
- Experience working with substance use, homelessness or social needs programs.
- Additional experience in excess of the Selection Requirements.

Additional Information Examination content :
This assessment type, weighted 100%, will evaluate the experience you list on your application, including responses to desirable qualifications the supplemental questionnaire and assign a score based on a standardized scoring system.
Applicants will need to achieve a passing score of 70% or higher on the assessment to be placed on the list.
Eligibility information :
Applications will be processed as they are received and added to the list accordingly for 12 months.
